Azalea Health Center is conveniently located to Select Specialty and Walton Rehabilitation Hospitals. We also accept referrals from Augusta University hospital and Piedmont Hospital. Working closely with Charlie Norwood Veterans Administration Hospital, we are proud to care for those who served our country in the United States military.
We offer a continuum of care on our campus, ranging from independent living at our Augusta Villas assisted living facility to skilled nursing and rehabilitation at Azalea Health Center. In addition to 24-hour skilled nursing services, we offer medical staff 5 days per week. Our dedicated rehabilitation staff are in-house and employees of the center. Azalea Health Center is a nursing agency free environment.
We offer a host of amenities including spacious resident rooms, free salon services at our Esteem Salon and a first class dining experience. We are also proud of our outdoor covered patio space and other scenic outdoor spaces.
Services
Azalea Health Center uses individual care plans to help seniors with their long-term care, skilled nursing, and rehabilitation needs. It has specialized care tracks, including cardiac and respiratory care. We also provide on-site laboratory and radiology services, and pain management

It is our goal to keep our residents, families and community informed regarding how we are doing during the COVID-19 pandemic and about the steps we are taking to keep our residents and staff safe from the spread of the virus. Language Assistance Services are available to individuals with limited English proficiency, upon request.
Dementia/Alzheimer's Care
Dementia is not one specific disease, but is a term used to describe a variety of symptoms caused by disorders that affect the brain. These disorders are characterized by impaired memory and difficulty with language, motor activity, recognizing objects and “executive function,” which refers to the ability to plan and organize. Alzheimer’s Disease is a progressive brain disease that slowly, over time, destroys memory and thinking skills. It is the most common cause of dementia among older people. If a family member or friend has dementia, you know that caring for them can be mentally and physically challenging, and can also put strain on your family and friendships. At Azalea Health Center, your loved one will receive around-the-clock care in a quiet, comfortable setting and be engaged in a variety of activities designed to provide a sense of self-worth and purpose. We also use music or memory programs to help patients enjoy each day to the fullest.
